Job description

Education and Credentialing Requirements: This position requires a minimum of a BA/BS in human services with one year of human services experience and/or an Associate in Arts Degree with three years’ experience in Criminal Justice or Human Service-related fields. The employee must be eligible and be registered with the North Carolina Addiction Specialist Professional Practice Board within 90 days of hire. The position requires the employee to obtain their CADC and CCJP within 5 years of employment.

Primary Purpose of Position: Provide Clinical Assessments, Treatment Matching, Referral, Care Management and Reporting. Exercise of discretion and judgment of diagnosing substance use disorder, screening for mental health criteria and placement of patient care. The creative ability to develop individualized person-centered plans and capable of intervening in client crisis.

Work Schedule: Full time professional position. General office schedule is Monday – Friday, 8 a.m. – 5 p.m., hourly 19.23. Hours may vary depending upon client and/or program needs. Evening hours are required to Facilitate Drug Education School and Recidivism Reduction Services classes weekly.
Other: Valid NC driver license is required. Travel is required and must be eligible for work related travel.
Duties and Responsibilities

  • Clinical Assessments, Treatment Matching

Screening, assessment, intake, orientation, and treatment planning

Screening clients for appropriateness to receive TASC Services. Responsible for diagnostic impression using DSM V criteria, developing appropriate level of care using ASAM criteria and developing a person-centered plan.

  • Referral and Consultation

Referring clients to community base resources for treatment and support services.

Matching clients with the appropriate levels of care

Position is responsible for facilitating the referral and maintain contact with the treatment provider.

  • Care Management

Monitor TASC client progress to include monthly contacts as required by the SOP.

Submit client status reports to appropriate criminal justice officials monthly.

Discharge clients upon successful completion of the program or for noncompliance.

Collect urine samples from TASC clients; keep records of urine samples and results.

Responsible for filing any forms, letters, etc. to keep client record updated.

Collect TASC fees and document as needed.

Attend weekly TASC staff meeting, and case reviews as needed or staffing as needed.

Conduct monitoring, evaluation and outcome studies and other quality improvement activities as required.

  • Report and Record Keeping

Responsible for medical record documentation per Insight Human Services, Inc. policy procedure manual and TASC SOP. This includes electronic medical records and web-based reporting system, such as NCTOPPS.

All services are provided in accordance with the North Carolina TASC Network Standard Operating Procedures.

Incumbent is responsible for following all State and Federal Confidentiality Laws.
