Position will be responsible for administrative and supervisory functions for a care management program coordinating whole person care with persons with behavioral health and physical health needs. The supervising care manager will develop and manage a team of care managers to provide whole-person care and foster high-functioning integrated care team approach to deliver better health outcomes.
Qualifications for the successful applicant include fully licensed LCSW, LCMHC, LPA, LMFT, an additional LCAS is preferred; three years’ experience providing care management, case management, or care coordination services for persons with behavioral health and physical health needs.
Responsibilities include:
- Supervising up to 8 care managers
- Review care management plans
- Provide guidance to care managers to ensure behavior and physical health care needs of members are met
- Coordination of services with other providers, both internally and within the community.
- Coordination of care managers to ensure 24-hour crisis coverage
- Promote System of Care strategies
